Unable to Add CF 4.5.1 Performance Monitor Counters in Windows 2000

Running ColdFusion Server v4.5.1 on Windows 2000 Advanced Server may produce a situation where the ColdFusion performance counters appear in the Windows 2000 Performance Monitor, but the Add button (to add the counters to the monitor) is grayed out. In addition, the cfstat command line utility may be unable to connect to the ColdFusion Server instance running on the server, making it impossible to gather ColdFusion performance data.
